We are excited you want to become a member! Every teen must fill out a registration form and mental health questionnaire before they can enter the facility.  It is highly recommended that you go over our contract/rules and read the FAQ in full before coming in.

 

Once these forms have been submitted, you will gain access to the facility and have a free 7-day period. Once the 7-day period expires, membership dues are due.

The BREAK Contract:

We thank you for adhering to the following policies created by your peers:
1.) Check in when you arrive and out when leaving
2.) Use appropriate language
3.) Clean up after yourselves
4.) Sign out equipment and return to proper location
5.) Respect other people's belongings
6.) Bullying and intimidating will not be tolerated
7.) Roughhousing and fighting are not allowed
8.) Respect all volunteers and staff

¡Nos entusiasma que quieras hacerte socio! Cada adolescente debe rellenar un formulario de inscripción, una renuncia a Covid y un cuestionario de salud mental antes de poder entrar en las instalaciones.  Se recomienda encarecidamente repasar nuestro contrato/reglamento y leer las preguntas frecuentes en su totalidad antes de entrar.

 

Una vez enviados estos formularios, tendrás acceso a la instalación y dispondrás de un periodo gratuito de 7 días. Una vez que el período de 7 días expire, se deberá pagar la cuota de socio.

4.) Firmar la salida del equipo y devolverlo a su ubicación adecuada
5.) Respetar las pertenencias de los demás
6.) No se tolerará el acoso ni la intimidación
7.) No se permiten las peleas ni los juegos bruscos
8.) Respetar a todos los voluntarios y al personal
